
The Outpatient Diabetes Self-Management Program offers comprehensive diabetes education and support services to outpatients and their significant others. The overall goal of the program is to provide the diabetic individual with self-management skills for control of their disease.
The program seeks to educate participants on the factors that affect glucose levels and how to implement appropriate lifestyle changes. Through application of medically sound self-management, diabetic individuals potentially reduce the risk of future health complications and significantly improve their quality of life.
The Outpatient Diabetes Self-Management Program is taught by a Registered Dietitian/Certified Diabetes Educator and a Registered Nurse with special training in diabetes education. Both are certified insulin pump trainers. The program is recognized by the American Diabetes Association. Physician referral is required for participation. Classes are conducted at the Wellness Center.
